Liability Waiver/Permission Agreement
In consideration of the foregoing, I, for myself, my executors,
administrators, and assigns, do hereby release, and discharge Evident
Ministries, Inc., and its directors and employees, and all sponsors,
assistants or volunteers from all claims of damages, demands, actions,
and courses of action whatever, in any matter arising or growing out of
my participation in said event. I certify that I have prepared myself for
this event and that I am in adequate physical condition to compete in
this event I have entered on Saturday, November 22, 2014. I agree to
follow all rules of this event and permit myself to be removed from
participation if, in the opinion of medical personnel, continuing in the
event would endanger my health. Further, I hereby grant full
permission to any and all of the foregoing to use any photographs,
videotapes, motion pictures, recordings, or any other record of this
event for any purpose whatsoever.
